In
accordance with the provisions of the Vital Statistics Act, the
birth of every child born in the Province of Ontario must be
registered with the Municipal Clerk of the municipality in which
the child was born within 30 days of the date of birth. There is no
charge for the registration.
In
order for the registration to be completed by the municipality, the
Statement of Live Birth must be accurately completed and submitted
to the Township. If the Statement of Live Birth is accurately
completed, the Township of Wilmot forwards it to the Province of
Ontario, Office of the Registrar General where the official
registration takes place. The Registrar General will take
approximately three (3) months to complete the registration and will
send a “Notice of Birth Registration”. Do not apply for a birth
certificate until such confirmation is received. You may download
Birth Certificate applications from this site, obtain
from the Clerk’s Office, or use

the “new” online birth
certificate application on the
Registrar General’s website.
If you wish to inquire about the status of a birth
certificate, please call 1-800-461-2156. Do not contact the
Township of Wilmot for this purpose as we are unable to
provide assistance.
The following information
applies only to parents who need to obtain a passport for a
child before a birth certificate is received. For this
purpose, the Clerk may issue a Division Registrar’s
Statement, also referred to as a “Travel Letter” for a child
born in Wilmot Township. A Travel Letter can only be
issued up to 90 days (three months) from the date the forms

are
received by the Township of Wilmot for the purpose of registration.
Such a letter can only be issued to the mother or father of the
child. The attending parent must have photo identification and be
able to answer key questions. There is no charge for the Travel
Letter.
It
is imperative that before obtaining a letter from our office, that
the Passport Office, appropriate Consulate, US and Canadian Customs
are contacted to determine whether the letter is sufficient to apply
for a passport, visa or to take the child across the US border. The
letter can usually be issued immediately upon attending our office.
